Your Lifestyle Medicine powered journey to health begins with a review of how you are doing today in the 6 areas of a health promoting lifestyle. 

·      Before we first meet you will be asked to provide some background information including your health history and most recent laboratory studies and to complete two assessments—an overall health related lifestyle assessment and a more detailed nutritional assessment. 

·      All of this information will be reviewed and organized to guide our conversation when we meet. 

During our initial conversation ( 80 minutes) we will review your baseline health profile and discuss the two areas of lifestyle in which you are most interested in making positive changes now. 

Based on our conversation, I will craft for you a plan with concrete steps for raising your level of wellness to support a longer HealthSpan.  

To conclude your initial consultation, we will meet a second time to review and refine the plan.

You and the health care providers you designate will receive both a report from your evaluation and your recommended next steps

—your personal HealthSpan Plan.

Couples and family consultations will include both individual and family level assessments and HealthSpan Plans.

Select a package of ongoing support

You get to set the pace, but between consultations you want to have time to really try out some of the changes you are committing to make, to see if they are realistic or need some retooling. 

Depending on your individual plan, you may be submitting information such as from a fitness tracker or nutrition app or a blood pressure or glucose monitor in between sessions.  The practice is not 24/7, but I will respond to this data and any quick questions you have.

Follow-up consultations should be fun and practical.  Since they are virtual, no matter where you are, we can go for a walk together while we talk, review what’s in your kitchen or go shopping.  Just about the only lifestyle skill we can’t practice together is sleep
